Pharmacist Health Coaching – Smoking Cessation Program

GSC’s Pharmacist Health Coaching – Smoking Cessation Program is a cognitive service provided by pharmacists which focuses on assisting patients in their efforts to quit smoking. The program is available to eligible GSC plan members and dependents.

Residents of Alberta and Saskatchewan, and Ontario residents age 65 and over are eligible only for smoking cessation drug therapy under the program as comparable counselling services are offered through their provincial plan.


Patient Forms

You may use your own forms to document your meetings and counselling sessions with patients. However, we recommend that the standardized template forms from the Ontario Pharmacy Smoking Cessation program or the Canadian Pharmacists Association (CPhA) forms be used as examples of the minimum level of documentation required. It is GSC’s expectation that participating pharmacists provide proof of completed documentation should an audit be performed for submitted claims.


How to Participate

In order to participate in GSC’s Pharmacist Health Coaching - Smoking Cessation Program, pharmacists are required to complete a smoking cessation training program before providing counselling services to patients. It is GSC’s expectation that participating pharmacists provide proof of completion of an applicable training program should an audit be performed.

Pharmacist Training

GSC has not developed specific smoking cessation training for pharmacists as there are a number of appropriate programs available through other organizations. We have provided links to some of these programs.

5A’s Algorithm

Pharmacists are expected to use the “5A’s” smoking cessation algorithm that is commonly used by health care providers to guide patients through a smoking cessation program.
